ACE Inhibitor or ARB for Left Ventricular Systolic Dysfunction (LVSD): Percent of eligible heart attack patients who received a prescription for ACE (angiotensin converting enzyme) inhibitor or ARB (angiotensin receptor blockers) medications to continue treatment for their heart attack when discharged from the hospital.

 

Smoking cessation advice/counseling: Percent of eligible heart attack patients given advice about stopping smoking.

 

Beta blockers at discharge: Percent of eligible heart attack patients who received a prescription for beta blocker medications to continue treatment for their heart attack when discharged from the hospital.

 

Discharge instructions: Percent of eligible heart failure patients given discharge instructions upon discharge from the hospital about their condition and follow-up treatment.

Assessment of Left Ventricular Systolic Dysfunction (LVSD): Percent of all heart failure patients assessed for their heart's ability to pump blood effectively.

Immunocompetent patients with community acquired pneumonia (CAP) given the recommended initial antibiotic: Percent of eligible immunocompetent patients with CAP who received the appropriate antibiotic to treat their pneumonia.

Influenza vaccination: Percent of eligible pneumonia patients who received an influenza vaccination during their hospital stay.

Received preventative antibiotic within 1 hour before incision: Percent of eligible surgical patients who received their preventative antibiotic within 1 hour of the start of surgery.

Recommended preventative antibiotic: Percent of eligible surgical patients who received the appropriate preventative antibiotic prior to surgery.

Preventative antibiotic is stopped within 24 hours after surgery: Percent of eligible surgical patients who had their preventative antibiotic discontinued within the recommended amount of time after surgery is completed.

Appropriate hair removal: Percent of eligible surgery patients having hair removed appropriately from the surgical site prior to surgery.

Recommended Venous Thromboembolism (VTE) Prophylaxis ordered: Percent of eligible surgical patients whose physician ordered treatments to prevent blood clots for selected surgeries.

Received appropriate Venous Thromboembolism Prophylaxis within 24 hours prior to surgery to 24 hours after surgery: Percent of eligible surgical patients who received treatment to prevent blood clots for the recommended amount of time for selected surgeries.
